VISIT CHECKLIST

When you come to your follow-up appointment to see Dr. Wong please remember the following: 

  • Patient forms 

  • Arrive 1 hour early for X-ray imaging at 1799 Robie St., Halifax, NS

  • Health card

  • List of medications. (*please report any changes to medications being taken)

  • Insurance information

  • If MRI imaging was conducted at a private clinic, a copy of the MRI disk provided
